Re: unpaid wages Dear [your supervisor's name]:
I am writing in reference to my paycheck. I was hired by your company on [your hire date]. I was to be paid a rate of [your salary]. I worked for [ number of hours] hours per day, and [number of days] per week. My last day of work was [last day you worked].
I have not received a check for [amount of time that you worked but were not paid for]. I worked those hours between [the dates you worked the hours you were not paid for]- I calculate the amount of wages owed to be [amount of money owed to you], based on my rate of pay and hours listed above.
On [date], I requested a check from [the name of the person you spoke to], but I have not yet received it. According to Arizona's Wage Payment law, I am entitled to full payment of the wages owed either within three (3) days from the date my employment with your company terminated or by the next pay period. I expect a check for the wages I am owed to be sent to me at the address listed below in accordance with the law.
